With an additional engraved title page with imprint: "London Printed for I.D. and sould by E.M. at the signe of the Swann in St. Pauls Churchyard. 1663."
An engraved chart of the humors, with illustrations of Oxford and Cambridge at the head is bound on a leaf between p. 76 (leaf F1) and p. 77 (leaf F2). At foot of leaf it reads: p. 77. In some copies this may be bound as a frontispiece.
Signatures: pi² [par.]⁸ A-K⁸ L⁸( -L8).
